We suspect that a form was deleted, which left issue in the design review to be link to an orphan form.
Request si to allow us to reassign orphan form to one that is active.
Generally speaking we would like to be able to change form in the lifecycle of an issue. As the issue mature or evolve to something bigger or more complicated than what is initially observe.
Or we simply have a new version of a form and wanted to move older form to migrate to the new one.
