Can we use a value from the document with the issue or folder that ahs the documents being reviewed to drive a value on the form?
Create a new attribute on the folder and then this value can be used to drive the form that is loaded or set a value on the form.
example.
Attribute used on the Folder for design review (Form = Design review) and one for discipline approval (Form = Discipline Review). More advanced reviews require a review spotlight, this could be a attribute of the document or folder where the form looks the same but the title would reflect what is the focus of the reviewed.
